Our continuum of individualized rehabilitation services assures the delivery of a level of care most appropriate for each person to maximize and maintain their potential in a setting specific to their needs. We expertly provide specialized rehabilitation services to adolescents and adults who are neurologically impaired due to:
- Closed or penetrating head injuries
- Cerebral vascular accidents
- Brain tumors
- Anoxic, and hypoxic encephalopathy
- Developmental disabilities
- ADD/ADHD
The following admission criteria has been established to assure maximum benefit is realized for those individuals participating in ACT, Inc. programs and services. The person receiving services:
- is 16 years old or older, with cognitive, physical, social, psychological, educational, or vocational deficits resulting from a neurological disability. We will consider providing services to those under age 16 on a case-by-case basis.
- should be medically stable and not require life support systems, skilled or intermediate nursing care. Person receiving services may need a prescription for services by a licensed physician updated annually or as required.
- has capacity for self-medication or medication with assistance.
- is not actively suicidal and has exhibited no history of such behavior in the past six months.
- should be independent in ambulation with or without assistive devices.
- should be able to complete activities of daily living (i.e. dressing and personal hygiene) with no more than moderate assistance.
- must be capable of self- preservation in residential programs.
- Appropriate funding must be available for the services provided.
